adding forms?

this site is for santa letters, and when you order, you need to include the childs name, friend,city, and a few others "custom" items to fit in the letter to make it customized for the perticular child.

I guess I will need to add some kind of form so the customer can fill in the information after they choose the letter style.

What why and where??????

You might be able to add those items as attributes to the product.

tried to get attributes, like figuring out how the pyramids were built.

cnat get them to show up, followed all the posts on adding, just wondered if a "add form " to an area?
