I use a modified Environmental Handling Fee download to charge various handling fees for size and weights of shipments. I had a problem that certain fees displayed at checkout just fine while others refused to display.

What I found is that when setting up the $ to apply to the fee description, all of the $ have to be a different denomination. As an example, 10 cannot be listed twice. The first listing associated with a 10 will be ignored and treated as "none". The second listing with a 10 will work fine.

My solution was to change one of the 10s to an 11.
