Additional customer fields

I have downloaded Additional Customer Fields module that is add-on section of the main zen site. But for some reason, as soon as I apply the sql patch, it gives me the following error:

1054 Unknown column 'field_date' in 'field list'
in:
[INSERT INTO customers_additional (field_id, field_name, field_date, field_required, use_url, field_url, field_status) VALUES (NULL, 'test', '2008-08-29', '0', '0', '', '0')]
If you were entering information, press the BACK button in your browser and re-check the information you had entered to be sure you left no blank fields.

Does anyone know how to fix it? Or maybe someone could help add an additional field into the customer registration page? I really need it! Please!

How did you install it? I tried to download the latest version and I get a .rar file not zip. I can't do anything with it.

You just need to copy the admin and includes folders to your zen folder. The Docs folder is just documentation on the mod. Please read the manual.ppt with step by step instructions on how to install the mod.

You might want to look at the (very long) support thread for this mod for information. Continuing the discussion there is probably better than scattering it in different threads.
It appears TheOracle came back to the forum in May '08 (after 11 months absence) to post another update which he says fixes all the bugs. I would want to verify that before trying to install it on a live site.

Is this the same MOD I would use if I want to add more fields for address book not on CUSTOMER INFO?

I wouldn't advise trying to use TheOracle's mod if you are not thoroughly competent with PHP. The thread mentioned in post 9 looks like a much more reliable method, albeit you have to do all the steps yourself. If you are careful, it seems you should be able to make it work.
