Yes I have installed the MOD as per instructions and I also get the same error:

1054 Unknown column 'field_required' in 'field list'
in:
[ SELECT field_id, field_name, field_required, field_status, use_url, field_url FROM customers_additional]
If you were entering information, press the BACK button in your browser and re-check the information you had entered to be sure you left no blank fields.

Of course the store also does not show. I looked at other threads but did not find anything on this error after installation.
