Can i add a feedback button to the sideboxes to link to a form?

Certainly.

Easiest way would be to create a new sidebox and pop your button and an associated text in there.

There are two free ads designed to make this easy for you. The Blank Side box if you are confident about inserting your own HTML for this, or the Editable Sidebox if you would prefer to use the WYSIWYG editor built-in to the Admin.
