Extra Customer Fields Mod seems broken

Hey everyone I installed the Module for Extra Customer Fields. First question, why is this module even available with all the warnings that are attached to it concerning Database errors, and database mess-ups? I decided to try it anyway and followed all instructions. When I go to the additional customer fields in admin I get the following error:
1054 Unknown column 'field_required' in 'field list'
in:
[ SELECT field_id, field_name, field_required, field_status, use_url, field_url FROM customers_additional]
If you were entering information, press the BACK button in your browser and re-check the information you had entered to be sure you left no blank fields.
Does anyone know that this means and if it can be fixed?

Hi

Did you upload all of the files associated with this mod? Check again. Is there a associated discussion forum for this link, have you tried asking there first?

Have you checked your database tables for the entries that should be there?

I to have the same problem and error after install. I have seen this same problem mentioned in a few different threads but I have yet to find a answer on why the error is there or how to fix.

I am wondering if anyone has installed this MOD without the error?
