Sidebox Custom Search

Hi,
I'd like to know if this is posible with Zen Cart, the best way to describe this, is with an image. :)
I'm not a PHP coder, so I would not know where to start, I could even trade CSS layout design & graphics for this mod.

I tried that, but it does not display on the sidebar even after I activate it under layout boxes controller, it's ON on my left column.

Any other suggestions?

That mod is meant to be sidebox, not integrated into header.
