user-customizable product addon?

I'm looking for an addon that will allow a user to customise a product with text and an image they can upload (e.g. a custom t-shirt). Does such an addon exist?

This is quite possible in zencart. Look in the TUTORIALS for "Attributes".

I see this tutorial

but that seems to be about attributes at the back end. I can't see how it allows a user to upload text/images when ordering an item.
