Credit card display

Hello Everyone!! Any reason why when I go to the checkout it asks me to put in my CC info twice? The first time it asks for:

Cardholder Name:
Credit Card Number:
Expiry Date:
CVV Number:

The second time it asks for:

Card Owner's Name:
Card Number:
Expiration Date:

Plus I can't get the images of the credit cards to show up?

If it's asking for Credit Card information twice, then you have 2 payment modules enabled which ask for credit card details.
Turn off any modules you're not needing.
