1.3.8a updates

I just downloaded 1.3.8a and just wondering what exactly are the (currently) most important updates (security, major bug fix, etc.) should I be putting for my default installation. I used a very old version before for a client and now needs an upgrade. Thanks.

IMHO, you should use version 1.3.8a as a whole and not take only parts of it.

Which means:

1. Create a fresh install of 1.3.8a as is (plus the security alert in this link).
2. Copy your client's customizations to the new install
3. Test it.

That is, are the fixes listed in that sticky thread already included in v1.3.8a?

The thread applies to all versions of 1.3.8. In fact, you'll see that the first couple entries specifically mention that they're fixed in 1.3.8a. All the rest you'll have to do yourself
