Table Rate Issues when based on Price (getting 0.00)

First post -- please help.

I've looked everywhere I can think of and haven't found a clue as to what I might be doing wrong here.

Installed Table Rate Clone mod according to instructions (but I was running into this issue before I installed the mod).

Issue:
Rate Table calculations don't seem to be working. Values keep showing as '0.00'.

Information:
I provided rate values as follows:
5:1.00,29.99:3.00,49.99:4.00,50.00:5.00

I selected 'Price' as table method. In one of the two tables, I tried multiple things to get it to work ... including putting $ in front of the amounts. Interestingly, if a % value is at the end (like the example in Admin) -- that with calculate (but nothing else will).

What I'm trying to achieve is a shipping charges as follows:

Order Total...........Shipping Charge
------------------------------------------
Up to 5.00..................... 1.00
5.01 to 29.99..................3.00
30.00 to 49.99................4.00
50.00 and up..................5.00


The second rate table is for upgraded shipping

Order Total ...........Shipping Charge
------------------------------------------
Up to 5.00....................5.00
5.01 to 29.99...............7.00
30.00 to 49.99.............8.00
50.00 and up...............9.00

Thanks in advance for your help. I'm sure I am doing something very basic wrong here... I just can't figure it out.
