Using Checkout Without Account Module

I just recently installed the Checkout Without Account Module and I'm getting a 1062 Duplicate entry '0' for key 1 error whenever I make another test transaction. But the error goes away when I click Refresh and then I get a Checkout Success message.

But then the order doesn't show up in the Admin, and I don't get an email alerting of a new order....

I am completely stumped. And this is for my job, so I am desperately needing some help.
