IT WORKS!!!!

Here was the problem after countless hours of checking everything, hopefully this post saves a lot of people time.

Letter Mail for what ever reason will not work if your shipping estimator is in "Popup" mode.

Once I switched it to "2= Display as Listing on Shopping Cart Page" inside of
Config/shipping/packaging/Shipping Estimator Display Settings for Shopping Cart

Everything works!

I have followed everything in this post and I cannot get this working. I have checked and re-checked the sellonline site, and my site and I am still not getting lettermail shipping showing up.

I had hoped that removing the popup item would work, but it didn't. I have setup a test product to use.

Hello Shanon,

I notice something. Can you change Large Envelope to Lettermail_1. Reason for that is. It has the same height as your Lettermail_0.
