Add customer from admin addon

I have this MOD installed and have a few problems with it.

1). The customer doesn't recieve a welcome email althought the box is checked
2). I can manually add an order for the customer but then stuck when getting to payment page. The customer has already paid via paypal (from an ebay purchase) so how would I reflect this in the customers account?

I have the same problem on issue 1.
I have read on other topics that the problem would be with FEC or easy signup but I dont have it installed....

So indead, anyone ?

I had the same problem (as number 10).
"Either the header row in the input file is empty or it was not recognised"
I even used the addresses.csv file as my template/model as well. Any suggestions?
