As title, i'm creating a seperate page with a custom PHP enquiry form.
Is it possible to remove the default form from the Contact Us page?

Vinyl Signs is correct. Change the setting to 3 and you should be ok.

Nope, that just hides the text that i've entered via Define Pages...
I want the actual form removing.

Either delete or comment out the form in your template or create a new page to use for contact purpose
