Could you peek at the settings in Configuration ... GV Coupons ...
Maximum Discount Coupon Report Results Per Page
Number of Discount Coupons to list on Reports PageMax sure the value is greater than 0 ...

If you do not see that setting ... did you upgrade recently?

Can you go to the Tools ... Developers Tool Kit ... and in the top input box enter:
MAX_DISPLAY_SEARCH_RESULTS_DISCOUNT_COUPONS_REPORTS

select NONE in the dropdown and click Search ...

Does anything come up?

I downloaded version 1.38 I have not updated.
I have the gv coupon as follows

Default Order Status For Zero Balance Orders Processing [2]
Length of the redeem code 10
New Signup Discount Coupon ID# 0
New Signup Gift Voucher Amount
Maximum Discount Coupons Per Page 0
Maximum Discount Coupon Report Results Per Page 0

in the developer tool kit, I get this

Key: MAX_DISPLAY_SEARCH_RESULTS_DISCOUNT_COUPONS_REPORTS

I want is to create a coupon that is free with the purchase of a quantity

Could you change the 0 per page on both of these to something like 10 or 20 per page so you do not have division by 0 errors?

I tried to add a new coupon but every time I save it the coupon name becomes blank and is saved as inactive coupon with the dates indicated above. I can't delete the inactive coupons too.

Please advise how will I resolve this.
