Does anyone know how to set up a basic form page on Zen Cart?

Basically, I' want to send out e-mails with a discount coupon which people can forward to a friend by clicking in a link in the e-mail.

From what I understand, the only way to achieve this is to have that link directed to a form on the site where they enter their friends e-mail address and it sends them a link to a static page on the site.

Anyone know how to set up basic form forwarding pages?
