Add Customers from Admin

Im having an issue when adding a new customer record thru the admin menu using this module:

When I enter company name and street address (plus all the other fields), the values in these two fields get swapped after the save.

The module works fine if a company name is not added and only a street address is used.

Thanks in advance for any thoughts.

Ive paged through it a couple of times and while it covers a lot of issues I didnt see ours in it?
