Two strange issues with Flat Rate shipping

I cloned the Flat Rate (FR) module as per instructions on the Forum and it plus the original are the only shipping modules I'm using. They work fine in adding the right amounts to the items depending on the Zone.

Issue A is that they do not add the tax to the shipping charges. The tax on the items work fine (since I started developing the site over two months ago), but not on the additional shipping. Yes, I selected the tax options in the FR module, I double-checked my tax rates and zones and priority and all that and still can't figure it out.

Issue B, perhaps related, is that both of the FR modules on the Admin Shipping Modules page are listed with yellow dots under the heading of Sort Order, not green dots as with a truly active module. (Their sort order is 0 and 1.) This is strange since, again, they are adding the shipping amounts to the items and seem to be active. All the other modules (Per Item, Per Unit, etc.) are red. I logged out and back in, rebooted the computer...and they're still yellow.

Does anyone know what's going on?
