Free/Flat Rate options not for everyone?

Hello everyone,
I am trying to dial in my shipping options on my shopping cart and I’m having a bit of a problem that I could use some help with.

We currently offer FedEx and USPS shipping options, as well as Free shipping on most orders over $99.00 and we have an $8.00 Flat Rate option for orders under $99.00

We ship worldwide and we have certain limitations as to who qualifies for or $8.00 Flat Rate option and our $99.00 and over Free Shipping option.

Basically it needs to work like this:

The $99.00 and up Free Shipping and $8.00 Flat Rate are for orders in the lower 48 states only. This means that Alaska, Hawaii and any other international locations should not have the option of choosing Free or Flat Rate. Alaska, Hawaii and all other international locations should only be able to choose from FedEx or USPS shipping options.

Currently if a customer enters their international address, my cart still offers them Free or Flat Rate. I have tried to turn this off by clicking on Modules/Order Total/Shipping then National. This has not worked.

What do I need to do to get my cart to not offer Free and Flat Rate to those customers that I don’t want to offer those options too?
