Discount Coupon Lookup not working?

I'm having trouble using the Discount Coupons look up

anytime i type in a code to look it up to check it, it just refreshes the page and does not display any coupon information!

It use to work fine, I am not sure what caused it to stop working as I don't recall making any changes!

The coupons are being created and seem to work fine. It's the look-up that is not working...

any ideas?!
