Hello, I'm using zencart version 1.3.8a and added the optional shipping insurance module, but must be doing something wrong because it's still not working. Can you help me figure out what I'm doing wrong?

Under "order totals" I've set "shipping insurance" to the following:
- set it to "true" so it's enabled (I thought)
- sort order: "400"
- table rates: "false"
- alternate insurance type: "amount"
- insurance %: "0"
- insurance rate" ".45"
- increment amount: "50"
- amount exempt from free: " " (no value)
- tax class: "none"
- no insurance fee on virtual products: "true"
- no insurance fee on free shipping: "true"
- required insurance amount: " " (no value)
- Insurance zone 1 & 2: "none"
