That tutorial is great! Just one question, where do I go to create the directories? I see where there is an option to choose which directory to upload the image to but how can I create my own directories?
You'll have to use your FTP program to do that.

I tried to add a folder under the images folder. It looked like that was where it was supposed to go but it doesn't show it as a category to choose? Am I supposed to upload the folder somewhere else?
Since you're referring to product images, then if you're wanting to add a subfolder named "bunnies", you would use your FTP program to open your /public_html/images folder and create a new folder there named "bunnies", like this: /public_html/images/bunnies
(**NOTE** the use of "public_html" above is assuming you already HAVE a public_html folder, and that you've got your Zen Cart site installed in that folder. Adjust the instructions to suit whatever folder your Zen Cart files are in)
