Using the Suburb field to collect county for Sales Tax

Zen Cart newb here... Love the Zen Cart app! Got a question in regards to using the Suburb field to collect the county for Ohio Sales Tax.

So, I've installed the Local Sales Tax Mod and relabeled the Suburb field to "County (OH only)" on my new user sign-up page. What I'm wondering how to do is:

1) make the "County (OH only)" appear only when Ohio (id 47) is selected and

2) then list a drop down of the available county names and

3) have that populate the database when the user signs up.

I don't know how to hide/unhide the field when Ohio is selected as the state. I also don't know how to create a drop-down box that displays the Ohio counties and populates the DB when the user signs up. Any help would be appreciated. Thanks!

I did this for Florida but wanted to avoid adding new fields or changing code as much as possible ...

When I set up a site for Florida I made all of the counties states and set up each with their own tax rates ...

I'd be interested in how you resolved Ohio ...

There's a local sales tax mod that may achieve this for you. I've used it for Arizona. It supports both county and city taxes, but needs a health warning if you use it for cities as relies on text matching for that level. Counties should be fine though.
