Changing fields in contact us form

Hi,

Im sorry if this has been posted before i have been searching and following what others say but i cannot locate my contact us form page for editing, i want to edit a field within the contact us form but i cannot find it anywhere, any suggestions of where it might be? Also in my admin>configuration i enabled the store address but how do i add the store name address etc to this?

To edit the php file which creates the fields...
includes/templates/template_default/templates/tpl_contact_us_default.php
Save your edited version to your overrides folder.

To edit the information that shows on Contact Us, use the Define Pages editor in admin under Tools.

To edit the store address...
admin/Configure/My Store, edit the Store Address and Phone line.
