Additional Order Form

A total newbee to Zencart, and so far well impressed. I've been working away on my shop for the last few weeks and I'm almost there.
Heres my query - I have one product that is different to all others - Its a personalised item and will require customers to give me specific details before I can process the order. I'm thinking what I need is to have an additional order form for this product only, with all the usual order info, as well as the extra details required. How and where can I do this? Basically, its a data collection form I need, which is tied in to the order form.
Or is there a more logical way to achieve this?
Bear in mind, I'm no programmer, so please try to be specific about which files need to be changed and where they are to be found.

Thanks, Stevesh. I suspected there might be an easy solution and attributes seems to be what I need. One other question. I've got my attributes all sorted and working properly, but the cart page they appear on looks a right mess. Attribute box titles are too big and bold, and the boxes aren't aligned correctly. Where do I go to change the layout and appearance of this page?
