Step 3 of 3 Checkout Billing address & Credit Card Info Problem

Can some one please help me fix this issue.
As you can see in the screenshot, in the step 3 of 3 of checkout,
the billing information field is blank. I want the billing address to appear here.
Or even better, the delivery address to appear there.
Some one help me how to do this. I've searched everywhere on
google and couldn't find the answer.


Also, looking at the credit card information, the fields are out of line.
It seems the system is not detecting the credit card type, so every value is pushed up one line. Can you fix this problem, or do I have to live with it?

Both the address problem and the credit card problem are *not* normal behavior on a fresh clean install that's not been altered by any addons.
Perhaps one or more of the addons you're using has broken something.

I'm using Zen Cart 1.3.8a
Its a fresh install, no upgrade.
No addons. I don't think.
Removed login, eway modules installed.

I spent hours searching forums and google and couldn't find the solution..
