Adding fields in the checkout

Hi,
I'm setting up a flower delivery store and there are 4 fields that need to be filled in by every customer during the checkout process. They are:

1. Can we leave the delivery without a signature?
2. Recipients Phone Number
3. Desired Delivery Date
4. Gift Card message...

What I wanted to do was to put these extra fields on the payment information page. Could someone please explain how to do this? Anyone? I've searched all over the place to find out but haven't seen anything yet.. Thanks!

Anyone?? it's driving me nuts. I know the general concept I think, I just need to know where to put things. If I'm correct, it's something like this:

1. Add the fields to the template and name the fields
2. Add the new rows to the database
3. Get the POST data from the form and insert it into the database
4. Modify the admin template to display the new data in the database

Am I on the right track? Anyone have any kind of simple example code?
