1.3.8a CUSTOM Template not working properly

Experienced with Zen Carts, been awhile since anything major.

I'm building a new Zen Cart online on a fresh 1.3.8a install here:

Created CUSTOM template:

templates/
CUSTOM/
common
css
images
templates
template_info.php

Ended up loading from template_default everything from css and images.

In theory, new CUSTOM template should look like Classic Green (plan on customizing from there).

Cannot figure out problem.

I've spent hours re-studying template info, including these links:

PS: I may want to go with the 100% screen width, but not knowing how it works.
