In my trophy shop I would like to add a button to the product info screen labeled "custom engraving". OnClick i want this but to open a popup/lightbox (prefer lightbox) that contains a form. In this form I need the standard input boxes and menus that modify font of each line of the customers engraving. Based on the input I would like to have labels created in a display written over an image of the plate style selected. I would then like the "add to cart" button on the form to transfer the item and the instructions to the cart kinda like the product options do.

If possible CSS would be great.

Any help is greatly appreciated.
