Extra Check boxes at check out

I'd like to know how I can have 2 check boxes (one for YES donate $25.00 ) and ( one for NO ) at check out and then display this on the order as Donate: Yes $25.00 or as Donate: NO
Could some one walk me through this?

For any one that runs into this post, here's the solution. I used the "Priority Order Handling" mod and modified the text and commented out the line that adds the extra fee to the order total. I activate the mod and now at check out I have given the user the option to select "Donate" as a check box, if the user checks the box and clicks continue to review order the amount is displayed below the subtotal but does not add the amount to order total. We are donating 25.00 from the original purchase, user is not charged the 25.00 that is to be donated.
