How do I add my search box to the top bar?

By default, a search box was in the top bar. I got rid of it somehow and I'd like it back. How do I get it back?

in admin
Tools > Layout Boxes Controller
and turn on search_header.php

When I do that I get two search boxes that show up, on in the header and one just below the header. How do I make it add just the one in the header? I'm not talking about the sidebox either...

In the Layout Box Controller, make sure that the sideboxes/search_header.php is set to ON under SINGLE COLUMN Status but set to OFF for the LEFT/RIGHT COLUMN Status.

The search_header.php is the only one that should be ON under SINGLE COLUMN Status.
