That's not a Zen Cart bug. It's a misunderstanding of how things work.
MANY payment modules place credit card fields on the checkout payment page.
1. If you're not using a certain module, click the "Remove" button, don't just set status to false.
2. If you're seeing modules/things you don't want, turn off ALL the modules, and then turn on the ones you want one-by-one and test the effects.
