I have searched the forum and add-ons but can't seem to find a module to do this.

I want a page customers can view/fill in to display feedback or leave feedback on the website and service we offer. I had a go at creating one in and ezpage but this would not work correctly. Are there any add-on modules to do this ?

Do you want to have
- a simple form like Contact Us where customers can write their own text what they think about your website or
- a Poll where you a ask question and customers can choose between answers, or
- a survey where customers have to answer heaps of questions and spend half an hour or more and they get a gift voucher in return?

1. Is integrated in Zencart. You just have to write some text to motovate people to give you feedback.
2. I think I have seem a free addon for this.
3. There is free adon. But you could use #################### to create your surveys, match you Zencart template and add a link on your website to the survey.
