Messed up price display

I attempted a code from a previous thread that placed your price and quantity box next to each other and I'm afraid I didn't back up my original document so my page is all messed up. Here's a looksy -

What do I need to do to get Item name with a description under it, then under the "price" column have price and quantity box next to each other?

I don't know which page you have modified . but you are using classic zencart ( read about custom template and override ) so you can simply replace modified page with clean page from zencart download.
