Error: Attribute Option and Option Value Do NOT Match - Attribute was not added -

Goal: Add attributes to existing products.

Background: I'm very familiar with adding attributes. We do a lot of custom orders based on attributes.

STEPS TAKEN:

1. Created Option Name, "Color" (without quotes)
2. Selected Option Name and created Option Value "Blue (dark blue)" (without quotes and assigned to Option Name "Color")
3. Selected product in Attributes Controller and selected Option Name, "Color" then Option Value "Blue (dark blue)"
4. After clicking on Insert the error "Attribute Option and Option Value Do NOT Match - Attribute was not added - Color :" is shown at the top of the page in pink.

MY TROUBLESHOOTING

1. I've created a new product and tried to assign the attribute---Same error
2. I modified the Option Value to shorter text---Same error
3. I removed the parenthesis from Option Value---Same error
4. I created a new Option Name "Colors" (added an "s" at the end and of course, no quotes). I created a new Option Value "blues" (no parenthesis and no quotes) and attempted to assign this new Option Name and Option Value to the product (the new product I created and the old product I was trying to actually use)---Same error
5. I tried to assign a different Option Name and Option Value to the product (new product created for troubleshooting and the product I actually need to assign attributes to) for example, "Cars" (an existing already in use attribute that is on another product) with a new Option Value (created for testing) "Pontiac"...assigned them to the product---Same error

The Option Name is set for Dropdown. I've gone over every step, slowly and meticulously. It leads me to believe that there is an error in the database. We are using Improved Attributes. We've been using this add on for over one year without problems.

Where do I need to look to fix this problem? As it stands we are no longer able to add attributes to products. Is there a limit to the number of attributes you can have? If so, how do we increase this number?

I did make one error where I deleted an Option Value (no correlation with the Option Name and Option Values we are trying to add to a product before deleting them first from the product. Would this cause a problem in being able to add attributes to a product?

I have searched the boards and Google for an answer but it seems as if people are getting this error through not knowing how to set up attributes. We are very familiar with Option Names and Option Values and how they must match and how you must select them matching in the Attribute Controller before adding them to the product.

Any help is appreciated.

When you say that you deleted an Option Value without deleting them from the product, how did you do this?

In phpMyAdmin ...

If you browse the table:
products_attributes table

... on the ones that you deleted, are those options_id and options_values_id still in the products_attributes table?

... if you browse the table:
products_options_values_to_products_options

do you see the options_id and options_values_id that you deleted in this table?

This does sound like perhaps it is bad data causing the issue, but it is hard to pin point this when blind to the actual contents of the tables ...

FIXED!

I do not know what caused the problem. I did learn this... we did not have Improved Attribute Controller installed. We moved servers and redid our web design. IAC was not reinstalled on the new server for the new design.

We installed it and now it works as it should. The errors have disappeared. It was not a database error but a coding error along the way that IAC fixed. DB error IAC would not be able to fix...corrupt is corrupt.
