Mod files always go in your custom template folders. If you are using the Classic template, that would be the folder to use.
But you should look at the tutorials for creating your own template, copied from /classic/ if you like that as a starting point.
/template_default/ is the complete set of basic template files, and you never use that as a template by itself, nor should you ever edit a file directly in a /template_default/ folder; copy it to your custom template folders and edit there.

:D Have you ever taken your car to a mechanic after hearing a strange noise, and when you ask him what's wrong he screws up his face, grimaces and looks at you in a funny way?
You kinda know the repair is going to be EXPENSIVE ! :shocking:
Fortunately, there are few (if any) car mechanics on this forum...
Those errors indicate a bad or corrupted file - probably damaged during the FTP process.
I think the culprit file is _hideCategories.php
Where does the _ come from?
My version of that mod does not have _hideCategories.php , only hideCategories.php
Try uploading the files again. Don't run the sql patch again because the database is a separate issue - probably not afected as these errors don't suggest a dbase error.

DELETE: ._hideCategories.php from your server and FTP that file again..

I'm going to tactfully suggest that before you dive headlong into the process of installing add-ons to your site, you learn how to use backup features - particularly the database, which is a "fragile" bit of kit that does not take kindly to excessive tampering.
Reparing file-based errors is quite straight forward, but fixing a corrupted database is the one area of your site that will result in "car mechanic" consequences.
Your host may provide a database backup feature... learn how to use it and always do a backup before running any SQL patches...

By the way... I trust you are aware that this module DOES NOT WORK on PHP version 5 and above...
What version of PHP is on your server?
