Zen Cart 1.3.8a w/ Patch: 1::

I have been anxious to use this module and tolday I deceided to give it a whirl.
I followed the install info, had to change the admin folder name for security reasons.

I am able to see the ADMIN Extras and REFFERALS area but the users are not being asked the How they heard about us? question at sign up. Located here: https://gelcandlecompany.com/index.php?main_page=login

Why is this not showing up? Now I can't get anyone to register without them being asked this question, your thoughts are appreciated.
