How did you hear about us module problem?

I stumbled into a problem installing this same module into a sister site of mine.
After installing the module it allows the user to fill the required "how did you hear about fields" but after clicking "submit" it just takes you to a blank screen "index.php?main_page=create_account" screen.

I did notice that the user successfully registered but they are not aware of it, just a blank screen.
The Extras and Refferals and Refferal Sources seems work ok in admin, but what concerns me is this blank screen.

Any help would be greatly apprciated.

Most likely, there is a file with space(s) or blank line(s) at the bottom following the closing php bracket ?> which will cause this error ...

A quick way to find this and other issues is to get the Debug Tool from the Free Software Add Ons and see what error logs it produces ...

You were right, I ran the debug tool and found this:

So I gathered that this older backup file called create_accountOLD.php was the culprit so I deleted this old file and it seems to be working like a charm, no blank page.

Is there any other tool I can use to make sure I don't have other problems in the background?

I installed this mod and everything seems to be working on the users end, but I have no way to see what they entered in admin. Where do I go to see what the user put in?

I found where it was located: Admin->Extra->Referral Sources

But when I open that it says number of other sources, and from there I click on that and there is 0 showing. Same when I do a test customer to check. Any ideas?

After the module is correctly installed.... Register and select a 'how did you hear about us' answer from the pull down menu. Then in your admin click on 'customers' and look at the'Referred by column'. You should see your newest registered customer at the top. hope this helps.. I installed it and it works nicely.

I am getting the same thing...did you find out how to see the Referrals? It is not showing up in the customer referral column. It states none. Everything shows up and the customer can choose from the drop down, I just can't find what they entered.
