Delivery option in Shipping Module

We offer local delivery and I have found no reference to a charged delivery item occurring during a business day. And I can't find an add-on for this purpose either. Any help would be appreciated!

Sorry. Sometimes my brain goes faster than my fingers on the keyboard.

What I'm looking for is a way to charge someone for my delivering an item to them. In the shipping module, you can use flat rate, UPS, Fedex, USPS, even have the customer come to the store to pick up, but there is no option for delivering to the customer. We charge a minimum of $5.00 for delivery, higher the further away it is. I need to know how to give that as an option to the customer.
