Recommend a friend pafe?

Can any one tell me if there is a mod for recommend a friend ( to my online shop) I know there is recommend a product but this is no good for what i need.

I need customers to be able to input firends contact & email details into a form

I had the same problem with "email to a friend" button. Before it was out of function, now it disappeared.
