I don't know where it would have come from. Yes, I just did the upgrade and the security patch prior to upgrading. I don't believe I've added any 3rd party mods...only just what came with the initial install. Maybe I'll be able to determine where it came from, but maybe not. Thanks for helping.

I did check that page and it was with the upgrade but wasn't actually published for some reason. I made sure that page was then published and everything works fine now. Thanks.

I assume by "page" you really meant "file"? and were referring to the admin html_output.php file?
I assume by "published" you're referring to some sort of software on your PC? What alarms me most about the word "published" is that Microsoft introduced that term in their FrontPage software, and others. And it's a BAAAD idea to use any MS junk, er, software, when working with PHP files..
