Question About Account Info Change

We are using: v1.3.9d. No upgrade, fresh install. The only addons we have is Image Handler. Only customize is we created a new theme.

We have searched the forum and tried different things to accomplish this:
We are looking for a way to add 2 required fields.
If we can put them in account, that would be great. If not, we would like them on checkout page whether item is free or pay.

We do not need fax number so can we change that to license number and then add the date of birth to be shown when account info comes up while checkout please?

We need both to be required, both to be on all checkout pages like address and name, etc.

Is there an easy way to do this please? We know html and .php but are just learning about zen.
