You seem to have the right code (at least what I see in our one TM cart) in the source. However, I notice that the URL is not showing the "standard" information. Have you added some SEO mod or something else that would have modified the URL?

Yes I added the CEON URI Mapping mod.

Let's try turning that off. Ceon is very good at what he does, but TM is very good at changing some of the strangest things that mod developers would never consider changing.
