Random Checkout Issue

My client is telling me that several times a week, they get a call from a customer who says that they submit their cart and are bounced back to the home page of the site without being able to check out. The items are still in their cart and nothing is processed. Since I can't duplicate, I'm not sure what may be going on. I've checked the logs and see nothing out of the ordinary. Dozens and dozens of orders go through normally without this issue.

No one has had this problem or any suggestions as to why it might be happening?
