Custom Form for Requests

I would like to create a custom from to allow users to my site to request a catalog wihtout having to create an account.
I have searched the forum, and i cannot find any answers that help me out.
I am a new user, without too much coding experience. So, the simpler the solution, the better.
In my industry, most if not all of the customers wish a hard copy before ordering form a online store, and, I am sitting on 200K copies of my Fall catalog that will not go anywhere without a proper form.
It does not necessarily have to write ot the database – and I think I would rather that it didn’t, since I have NO idea how to do that. Something that send an email, or just dumps to a CSV file on the server would be the option hat I would go with.
I am trying to capture Name, ADDR, CTZ, email, & Phone, and that is all. (Seems simple enough to describe, but it is paining me to no ends to try it implement it.)
Some have posted changing the built in form, other 3rd party applications, etc. And I an honestly confused about where to turn and what to do.
Any direction for this would be appreciated.

Not an upgrade, initial install
No add-ons have been installed
No customization, other than the application of a template
Hosting is GoDaddy, No SSL (YET)

Can google for form as the code is standard
