checkbox attribute not displaying

Hello,
I'm trying to add a checkbox attribute to an item. Basically its a ticket and I want to have an checkbox option for the customer to select whether to mail it or recieve at registration desk.
Currently the item has a working text box attribute attached to it, but the checkbox attribute only displays the Option Name of "Ticket Delivery".
I should see 2 checkboxes.

Within Attributes Controller, I can click on preview attributes and it shows both the checkbox and textbox attributes, but doesn't work on the site. Here's a link to the item:

I'm running 1.3.9f
I do run Products w/ Attributes Stock module if that matters.

Did you make the Option Name a checkbox or text type?

When you added it to the Product, did you click on:
Guest 1

and then on the Option Value for it and click Insert ...

Or, did you click on the Option Name:
Guest 1

and "read" it on the screen and click Insert?

I made the Option Name a checkbox Option Type.

Guest1 is a separate attribute, textbox, that is working fine.

Just can't get the checkboxes to show up for the Ticket Delivery attribute.

Go to the Option Name Manager ... and find the Checkbox Option Name that you made and make sure it is set to Checkbox ...

Go to the Option Values Manager ... and find the Option Name that you made and make sure that you have created an Option Value Name for it ...

Go to the Attributes Controller ... and edit the Checkbox that you have made ... do you see the Option Value that you made for the Option Name selected in the Option Value box?

Under Option Names I have:
ID Option Name Type Sort Size Max
19 Ticket Delivery Checkbox 0 32 32

Under Option Values I have:
ID Option Name Option Value
15 Ticket Delivery via Mail
16 Ticket Delivery via Registration Desk

Under Attributes Controller I have the attached image. Am I missing something?

Do you have some type of add on that affects attributes in any manner?

Yes, I'm running the latest Stock by Attributes module. I mentioned that in my original post.

Sounds like something might be broken by using the Add On ...

I cannot reproduce this issue with a clean Zen Cart ...

Might check on that Add Ons support thread and see if you have left something out like perhaps you have no stock on that Attribute?
