Option name doesn't show up in order screen

Hi! I'm pretty new to Zencart and hope someone can help me. I added 2 new text attributes to my store. The are called Line 1(s/c) and Line 2(s/c). They show up for the customer and the information is collected, however when I view the customer's order (and the order receipt), the attribute name does not show up. So it looks like this:

- Size: Small
- : customer text
- : customer text

At least it does capture the customer's text, but I would prefer it to look like this:

- Size: Small
- Line 1(s/c) : customer text
- Line 2(s/c) : customer text

Does anyone know why the attribute option name isn't showing up? Thanks!

Sure, I am using v1.39e of Zencart.
Never been upgraded.
The only addon I have is the XML Sitemaps.
I use a template.
I use Bluehost.
I have only seen the problem as of today, but it was the first time I got an order on this particular product with these attributes.

This reminds me of a symptom of an old bug that was in v1.3.8 with either a template or an add on but I cannot remember what it was ...

Have you an URL to this Product that we could perhaps look at?

This appears to be working where I can see everything on the product and in the shopping cart ...

Can you test this by switching to the Classic Template and see if everything completes correctly?
