Orders not showing

Hi,

Recently we had a few orders but nothing was displayed in admin panel but payment went through.

I asked customer and he said during checkout he was never asked to add any details and then he was taken to Payment server page where he made payment.

It has happend twice. What could be the issue?

I have not added any module for the last 3 months so no changes have been made.

I have just had the same exact problem.... I get a payment via PayPal indicating a purchase was made through the website, but no order shows up in the Admin. My Zen version is1.3.7 and has been in place since 2007. This is freaky! Anyone's help would be appreciated.
