I have sorted out this issue this morning.

I have recreate copy the file and it works perfectly on 1.3.8 , but I am not sure if that work on 1.3.9

and if look for more zones, is similar to the Multiple Shipping Zones Rate Module readme instructions.

Hi Timlo: I have try, it is working, but how to add up to $this->num_zones = 60;
Cos your solution only allow add 3 zones.
